Privacy Notice on the Processing of Personal Data

1. Data Controller

This Privacy Notice has been prepared by Op. Dr. Javidan Asafov, acting as data controller under the Turkish Personal Data Protection Law No. 6698 (“KVKK”), regarding the personal data processed in connection with appointment requests submitted through this website.

2. Personal Data Processed

The following personal data may be processed through the appointment request form on this website:

• Name and surname
• Phone number
• E-mail address (optional)
• The procedure, treatment or subject of interest
• Date and time the request was submitted

No health data or detailed medical information is requested through the appointment request form.

Please do not share your medical history, diagnoses, test results, prescriptions, imaging results or any other special categories of personal data through the form.

3. Purposes and Legal Grounds of Processing

Your personal data is processed solely for the purposes of:

• receiving and assessing your appointment request,
• contacting you,
• scheduling a suitable appointment date and time,
• providing the information necessary in relation to your request.

This data is processed on the following legal grounds set out in Article 5 of the KVKK:

• processing is necessary because it is directly related to the establishment or performance of a contract, and
• processing is necessary for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject.

The appointment request form is not intended for the processing of health data.

4. Transfer of Personal Data

Information submitted through the appointment request form is transmitted, via the technical infrastructure on which the website is hosted, to the e-mail address randevu@javidanasafov.com belonging to Op. Dr. Javidan Asafov.

Hostinger services are used for the hosting and technical infrastructure of the website.

Hosting, e-mail, security and similar technical service providers may access personal data only to the extent necessary to provide the relevant services.

Where the technical infrastructure used involves the processing or transfer of personal data via servers or service providers located abroad, such operations are carried out within the framework of the provisions of the KVKK on the transfer of personal data abroad and the applicable legal safeguards.

Your personal data is not shared with third parties except where required by legal obligations, duly submitted requests of competent public authorities and institutions, or where necessary for the provision of the technical services used.

5. Retention Period

Personal data and e-mail records relating to an appointment request are retained for 3 months following the completion of the appointment process.

At the end of this period, unless another legal obligation requires the data to be retained, the relevant personal data is deleted, destroyed or anonymised.

The content of the appointment form is not permanently stored on the website server.

To ensure the security of the website and to prevent automated or abusive requests, pseudonymised technical records derived from the IP address with a server-side secret key (they do not contain the IP address itself) or similar security data may, where necessary, be kept temporarily; these records are deleted automatically after 24 hours.

6. Method of Collection

Your personal data is collected electronically when you fill in and submit the appointment request form on javidanasafov.com.

Submitting the form is entirely at your discretion.

7. Your Rights under the KVKK

Under Article 11 of the KVKK, you have the right to:

• learn whether your personal data is being processed,
• request information if your personal data has been processed,
• learn the purpose of processing and whether the data is used in accordance with that purpose,
• know the third parties to whom your personal data is transferred, domestically or abroad,
• request the rectification of incomplete or inaccurate personal data,
• request the deletion or destruction of your personal data where the conditions set out in the KVKK are met,
• request that rectification, deletion or destruction be notified to the third parties to whom the data has been transferred,
• object to a result arising against you from the analysis of processed data exclusively by automated systems,
• claim compensation for damage arising from the unlawful processing of your personal data.

Requests are assessed and concluded in accordance with the procedures and time limits set out in the applicable legislation.

8. Cookies and Analytics Services

In addition to strictly necessary technical cookies, analytics cookies and similar technologies may be used on the website only if the user gives consent.

Analytics services such as Google Analytics are not activated without the user's consent.
